At Severn Vale Home Care, we can help you achieve both. For palliative care or end of life care, you will need help and support as health declines.
The challenge can be knowing if staying at home or going to a nursing home is the right decision. We believe that leaving your home, when you require palliative care, can be more stressful. This level of change would be difficult for anyone. A care home can also be more expensive.
We believe that home comforts should surround someone at the end of their life. We empathise with the pain of seeing a loved one suffer and wanting to ensure their comfort during their final days. Helping them through the pain they are suffering is a big responsibility. You want to ensure they best possible quality of life in the current situation.

- Decide to call us and learn more information about how we can help you. We have a small team of people who are friendly and available to talk and listen to you over the phone.
- They will come and visit you in your home and do a care assessment. Within 24 hours of the visit they will provide you with a care plan for you to consider.
- If you decide to continue with your care plan, a Care Professional will come at the agreed-upon times and days.
If you experience any concerns or problems with your care plan, our office team is available to listen and help. We are as sympathetic and understanding as possible to our clients views and needs.

Diagnosis of a life threatening condition
Severn Vale's healthcare professionals use a companionship-led approach to care. This helps you and your family make the most of your time together.
You always search for high quality specialist care out of necessity. We can help if someone close to you receives a diagnosis of a serious or life-limiting or terminal illness. For example you might be receiving cancer treatment and in these circumstances it might be essential to receive palliative care. This approach can help meet physical, emotional, and mental health needs.
Each circumstance is personal & unique. That is why your care service starts with listening to you, to understand your individual situation. This helps us to tailor a care plan whilst working with other health professionals that is as personal as you.
Your familiar possessions and all the happy memories associated with them will surround you. This approach allows personalized care to maintain many of your routines and preferences, meeting your needs and wishes.
Our health and social care professionals are reliable and highly trained in the care and support they offer. They can offer specialist support tailored to the needs of service users. We provide visiting care visits up to several times a day. We also offer 24/7 live-in care at home instead of a nursing home.
Our care professionals receive training in palliative and end of life care. Some members of our team have a specialist interest and receive additional training. Our palliative care teams will stay in contact with your GP, physiotherapists, and other professionals. This ensures that your care is up to date and consistent.
Our family run care service invests in retaining and rewarding our care professionals. They receive support from our friendly and caring management team. The team is just a phone call away, providing you and your family with complete peace of mind.
Our tailored approach to care, focuses on helping you to remain as independent as possible. Your care might involve administering medication, personal care, perhaps helping you with washing and dressing.
Your carer can cook the meals you like. They can also clean and tidy your home, if necessary.
Additionally, they can run errands and go shopping. We can also help you to stay as mobile as possible both inside and outside your home.

Care calls for Visiting Care can take place between 7am and 10pm everyday.
Live in Care is 24/7 and your Care Professional will require a daily break of 2 hours. There
are various options to enable this that will be discussed at the care assessment.
They will help with all personal care needs, washing, dressing, assistance to access toilet
facilities, meal preparation, basic housekeeping, administering medication, social trips /
appointments and looking after pets. Our Care Professionals are interesting and engaging
and also offer great company!
If you are receiving Visiting Care we will introduce your primary Care Professional who will
provide the majority of your care. We will also introduce you to a small team of Care
Professionals who will provide your Care when your primary care is on holiday or is
unavailable.
For Live in Care you will have one main Care Professional. We will arrange cover when your
main Carer goes on holiday.
Following an initial conversation we can usually accommodate a care assessment in 24 hours.
Following this we will discuss and agree a start date.
Our office team are in constant communication with your Care Professionals both prior to
your care beginning and throughout your care. Both our management team and family
members can access all care logs completed by our Care Professionals that is accessed by the
Severn Vale App. We also conduct regular quality reviews. Our friendly office team can be
contacted 7 days a week between 7am and 10pm should you wish to speak to us about any
aspect of your care.
If you have any concerns whatsoever you should contact a member of the Severn Vale office
team and we will help to resolve any issues.
Following the Care Assessment if you would like to go ahead, we will go through and leave a
copy of our terms and conditions for you to read. To begin Care we need a signed copy of the
Ts & Cs. You will also need to complete a Direct Debit form. We will send invoices
fortnightly in arrears and these amounts are collected usually a week later by Direct Debit.
For Live in Care we also take a returnable deposit equivalent to 2 weeks of care.
